Whilst we’ve received information that all 50-64 year olds may be eligible for a free flu vaccination as part of plans to protect more people against seasonal flu, these free vaccinations will be subject to vaccine delivery later in the year.

This allows us (GP’s and community pharmacists) to focus on those who are in an at risk group, such as pregnant women and people with long term health conditions. They will be given priority and offered the vaccine first; if you are in this category you will be contacted.

If you are aged 50-64 and not in one of the at risk categories you will be made aware in the coming months if you will be able to attend for vaccination. If the programme is extended this is likely to be November or December.

Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic ID.

You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. A registration form can be downloaded by clicking on the link below.

Medical treatment is available from the date of registration. Please contact reception for further information.

To register please come to the surgery and complete the necessary registration forms or alternatively you can print them off yourself at home. These are available below and both need completing in order to be registered at the surgery. Once completed, please bring to reception along with some photo ID (or red book for a baby) and proof of address (e.g. utility bill).
